Dear Incline Village & Crystal Bay Property Owners,
 
Thank you to all for sending in your W-9 forms as we requested.  However, some were submitted without a
signature or APN (parcel) identification.
 
If you have more than one property
that will be affected please send a W-9 form for EACH parcel.
 
The APN should be listed in the
space for List account number(s) here
(optional).
 
Be sure to sign and date the W-9 form where indicated.
 
The Treasurer is planning to return the W-9 forms that have missing
information for re-submittal.  If you
feel that yours was submitted incorrectly, I would suggest that you immediately
re-send a new W-9 form to avoid
confusion.
